Newly issued (within 6 months) BAPTISMAL and CONFIRMATION CERTIFICATES of the groom and the bride from the church where they received the Sacrament, with the inscription: FOR MARRIAGE PURPOSES ONLY
Certified True Copy of the Birth Certificates
Marriage License (Valid for 120 days) issued by the Civil Registrar of the city or town of either the groom or the bride. To avoid expiration of the marriage license, apply for the license preferably two months before the wedding. If the couple has been married civilly, a copy of the MARRIAGE CERTIFICATE from the Philippine Statistic Authority. If the couple has been living together for at least five years, four copies of an AFFIDAVIT OF COHABITATION
SINGLE STATUS CERTIFICATE (CENOMAR) of the groom and the bride issued by the Philippine Statistic Authority
Two (2) copies of newly taken 2x2 ID Photo (within 6 months) of the groom and the bride
Canonical Interview of the groom and the bride to be conducted at least one (1) month before the wedding the date
Ecclesiastical Banns (Announcement/Publication of the marriage in the parishes of the groom and bride for three consecutive Sundays) and permission to be secured from the parish of the couple. The appropriate form will be provided by our parish office.

It is highly recommended that the groom and the bride receive the SACRAMENT OF PENANCE (Confession) for the fruitful reception of the Sacrament of Marriage
